Reserving a Space for the Trip

Graduate Students Only. Program is now open for sign-ups.

To participate in the trip, send Dr. Sebastijanovic (msebastijanovic@uh.edu) an email requesting a spot in the program. Dr. Sebastijanovic will email you a confirmation of your reservation. Upon receipt of all completed sign up materials (see below) a non-refundable $300 deposit fee will be charged to the student's account within 14 days. Students are accepted into the program upon payment of the deposit fee.

SIGN-UP MATERIALS

To secure your space in the trip, students must submit the following materials to Dr. Sebastijanovic. Please note that some of the materials must be notarized:

  1. Sign-up Checklist
  2. Faculty-Led Student Information Sheet
  3. Learning Abroad Release and Indemnification Agreement Form
  4. Learning Abroad Image Release Form
  5. Proof of Registration with Smart Traveler Enrollment Program (U.S. citizens only)
    • Create an account
    • Register your trip with all details
    • Print and submit a screen shot of your profile, which must include your name, destination country, and travel dates
  6. Photocopy of your passport or passport application. Passport must be valid and not expire within 6 months of travel.
  7. Form A-1: Health Disclosure and Medical History. You must have health insurance that covers travel abroad to participate.
  8. International Student Identity Card
  9. Form S-2: Student Conduct Agreement Form. Sign and date.
  • Hard copies of these forms may be turned in to Dr. Sebastijanovic in 315 K Melcher Hall, or students may email them as one .pdf file to msebastijanovic@uh.edu.
