Accounting Certificate Request Form

BBA/CAP (O&G and IA)

This form is to be completed and submitted once you have completed all the coursework required for the certificate. Please indicate which certificate you are requesting and select the course, grade, and semester for the courses you have completed. To earn the certificate, you must:

Earn a passing letter grade for each certificate course.
Have a minimum cumulative GPA of 3.0 for the certificate courses.
Have a minimum cumulative and accounting GPA of 2.5.
Earn a letter grade of “S” (satisfactory) for the required colloquium.

Certificates will be processed for printing 30 days from the last day of the semester. If your request has not been submitted within this time frame, your certificate will not be processed until 30 days after the end of the following semester.
