Reserving a Space for the Trip

Graduate Students Only. Program is now open for sign-ups.

To participate in the trip, send Dr. Sebastijanovic (msebastijanovic@uh.edu) an email requesting a spot in the program. Dr. Sebastijanovic will email you a confirmation of your reservation. Upon receipt of all completed sign up materials (see below) a non-refundable $300 deposit fee will be charged to the student's account within 14 days. Students are accepted into the program upon payment of the deposit fee.

SIGN-UP MATERIALS

To secure your space in the trip, students need to submit the following materials to Dr. Sebastijanovic via email msebastijanovic@uh.edu as one PDF file.

REQUIRED
  1. Sign up checklist
  2. Image Release Form
  3. Release and Indemnification Agreement Form
  4. Student Code of Conduct Affidavit
  5. Photocopy of your passport or passport application. Passport must be valid and not expire within 6 months of travel
  6. Health Disclosure Form
  7. Proof of international health and emergency insurance
